QUESTION: I am a new employee, will the Plan cover my dependents automatically or must I pay for that coverage? ANSWER: The Plan covers eligible dependents of employees at no difference in the contribution rate.

QUESTION: How do I add my dependents? ANSWER: You can add a dependent by following the instructions on the Family Enrollment Card (FEC). You can obtain an FEC here. The FEC and any required documentation must be submitted to the Fund Office for final determination.
QUESTION: Why do you need my marriage certificate or divorce decree/court order? ANSWER: The Plan may require this documentation to properly determine coverage dates and also insurance responsibility for dependents.
QUESTION: My child is attending college this fall, will she still be covered under the Plan? ANSWER: Eligible unmarried children (ages 19-24) who are full time students may be eligible for coverage. The Fund Office will require periodic verification from the college.
QUESTION: My child is totally disabled, will he still be covered under the Plan? ANSWER: If your child becomes totally disabled prior to age 19, they may remain covered as long as you are eligible, annual verification is required. Please contact the Fund Office for review of the situation.
QUESTION: I am getting divorced, can my former spouse make self payments to continue coverage? ANSWER: Yes, ex-spouses can be entitled to COBRA (continuation) coverage if the Plan is notified within the proper time frame. Please contact the Fund Office if you become legally separated, divorced or no longer have a stepchild living in your household.
