You and your spouse should both carry a Blue Card (BlueCross BlueShield) I.D. card. Show your card to the hospital admissions office and whenever you or a family member receives medical care at a doctor's office or medical facility. Information regarding Mental Health and Substance Abuse Benefits and precertification by ValueOptions is on the back of that card.
Use your Express Scripts I.D. card for short-term prescription drug purchases at participating pharmacies. This is a requirement for Medicare-eligible participants.
When the Fund Office is informed that your or a dependent's coverage is going to terminate, it is required by law to send you information about your right to make self-payments. Therefore, the Fund Office should always have the current mailing address for you and all your covered dependents so that you can be sent this information as well as other notices which are mailed to Plan participants from time to time.
Be sure to let the Fund Office know if you or any of your covered dependents have a change of address. You can either: (1) call the Fund Office with the address change, (2) fill out a change of address notification on the Lineco website, or (3) fill out and mail in one of the change of address cards in this booklet. To avoid lost prescriptions, you should also inform Express Scripts—you can phone in your change of address to Express Scripts at 1-(877)-327-0568.
